How do you currently keep track of everything you learn about each student throughout the year?

Currently have a friend who is swarmed with this and is facing issues with managing things like observations, assessment results, behavior or notes, parent communication. Especially the students progress over time which parents care deeply about.
Is this information mostly in one place or spread across documents?
