What are some classroom management strategies for defiant students (excessive talking, refusal to work, up out of seats, not focused)?

I am constantly reviewing my classroom expectations and consequences. However, nothing seems to work. I am given silent lunch, called parents, completed office referrals, students have had after school detention, written reflection papers, met with parents, and some have even been given in school suspension and out of school suspension. I feel like I have run out of options. Any strategies or ideas? This is my 17th year and the first time I have had to deal with such defiant behavior.
