Learning Targets and Assessment

My school is adopting the Learning Target model this upcoming school year. There are many ideas regarding what this looks like in the classroom, but our principal wants us to focus solely on student reflection after assessments. The idea I have so far for managing the test and reflection is that students would keep these materials in a folder/portfolio. Students can then access previous tests and then use them to prepare for future assessments. My question is, does anyone currently use any sort of organizational method with tests and reflection that has worked well? Also, any suggestions on managing students after a test that need reinforcement or enrichment activities? Did you create the activities or find them on a particular site?
